Please bring the following documentation to your scheduled meeting:
" Proof of Identity for of household (Social Security Card, License, Passport, etc.
" Social Security Card or number for EVERYONE in the household.
" Proof of Address (current utility bill, income tax form, cable bill)
" Proof of Income for EVERYONE in the household (wages & salary-last four pay stubs, social security, SSI, TAFDC, EAEDC, veterans benefits, pensions, unemployment wages, wages from self employment, workman's compensation, alimony & child support, interest income over $100 a month, dividends from and bonds, support from family and friends, annuities, estate or trust income, rental income and cash prizes from winnings)
" Most recent heating, electric and telephone bill (Only Verizon) to quality for utility discount.
Housing Documents
For Homeowners
" Submit current mortgage statement, real estate tax bill and home owners insurance
For Renters
" Submit a current completed copy of signed lease
Renters with Subsidized Housing Assistance
" Submit a current income worksheet/tenant profile from the Housing Authority with lease contract
Tenant at Will
" Call the office for a Rental Information Form
The above are state requirements.
The applicant is also required to report all household members.
