Obtaining Copies of Vital Records

BIRTHS - The Town Clerk’s Office maintains birth records of births that occurred in Wareham and birth records of individuals whose parents resided in Wareham at the time of the birth. Certain records are restricted from public access. Identification may be required for processing some requests. Certified copies may be obtained in person for a fee of $10.00 each. Certified copies can also be requested through the mail. Please contact the Town Clerk’s office for instructions on how to obtain a Certified Birth Record through the mail. DEATHS - The Town Clerk’s Office maintains death records that occurred in Wareham and death records of those individuals who were residents of Wareham at the time of their death and that the death occurred in the Commonwealth of Massachusetts. Certified copies may be obtained in person for a fee of $10.00 each. Certified copies can also be requested through the mail. A written request should be submitted to the Town Clerk’s office with the induvial(s) name and date of death, along with the fee and a self-addressed stamped envelope. A self-addressed stamped envelope MUST be included with the request in order for the record to be mailed back. If a return envelope is not enclosed, the record will not be mailed. MARRIAGES - The Town Clerk’s Office maintains marriage records of those who filed their marriage intentions with the Wareham Town Clerk. Certified copies may be obtained in person for a fee of $10.00 each. Certain records are restricted from public access. Identification may be required for processing some requests. Certified copies can also be requested through the mail. Please contact the Town Clerk’s office for instructions on how to obtain a certified copy of a marriage license through the mail.

ONLY CERTIFIED COPIES are issued by the Town Clerk’s office, regardless of intended use.

Please note: A self-addressed stamped envelope MUST be included with any written request via the USPS in order for the record to be mailed back. If a return envelope is not enclosed, the record will not be mailed.
